3M INSPECTION NOTICE – IMMEDIATE ACTION REQUIRED

3M™ PROTECTA® Rebel Self Retracting Lifeline (CSA Versions)

Read on for an important 3M Fall Protection inspection notice – 3M™ Fall Protection has identified a potential manufacturing issue with a limited number of 3M™ Protecta® Rebel Self-Retracting Lifelines (with galvanized or stainless-steel lifelines) produced between October 14, 2019 and February 25, 2020. This manufacturing issue could result in the SRL not engaging properly but can be easily detected through the pre-use inspection as specified in the Protecta® Rebel Instruction for Use (IFU) document.

Given this information, 3M Fall Protection is issuing an “Inspection Notice” for the 3M™ Protecta® Rebel Self-Retracting Lifelines (with galvanized or stainless-steel lifelines) produced between October 14, 2019 and February 25, 2020.

End Users: Please follow the steps listed below.

Step 1: Locate the label on the Rebel SRL to identify the manufactured dated (see picture to the right). If the SRL has a manufacture date of 19/10 (October 2019) through to the end of 20/02 (February 2020), continue to step 2. (Please note that regardless of the manufacture date, all SRLs should be inspected prior to every use and by a competent person annually as per the IFU).

Step 2: Pull the lifeline quickly to ensure that the SRL locks up. As per the IFU “Ensure the device locks up when the lifeline is jerked sharply. Lockup should be positive with no slipping.” It the SRL locks up properly and passes all other aspects of the pre-use inspection as defined in the IFU, the SRL is acceptable for use. (For a full listing of inspection criteria please refer to the IFU for your respective regions which can be found at www.ProtectaRebelInspect.com). If you find that your SRL does not lock up, take the Protecta® Rebel SRL out of service immediately. Please contact our Customer Service department at 1-833-998-2243 or email us at 3mcafpserviceaction@mmm.com and we will arrange to have the SRL inspected and repaired/replaced as per our standard warranty.

Distributors: Upon receipt of this Notice, please contact our Customer Service department at 1-833-998-2243 or email us at 3mcafpserviceaction@mmm.com for a listing of the affected Protecta® Rebel SRLs sold to you. If you have any of the affected parts in stock, you should return them to 3M Fall Protection for repair and/or replacement as per our standard warranty. Please forward this Notice to any of your customers who have purchased affected products from you and provide any assistance requested by your customers to complete the process.

Please note they are not aware of any accidents or injuries related to this condition.

The Precautions We Are Taking 

  • Classes are limited to 4 students per class (to stay under provincial guidelines of 5, including the instructor)
  • A medical questionnaire must be completed before attending
  • Spacious classrooms ensure that no student is within 6 ft of one another
  • All touchpoints are sanitized multiple times a day including before students arrive, at lunch, and at the end of the day.
  • Dedicated washroom facilities for attendees that are sanitized and not used by staff
  • Providing gloves and (upon request) sanitized PPE for practical course segments
  • Very limited staff on-site that remain in their own offices

Instructor-Led Online and Blended Learning 

We are excited to be offering remote online learning courses via Zoom (a free video-conferencing application). Our talented Instructors will lead these courses and will be able to be seen and spoken to throughout the course.

Blended Learning

A small segment of our course offerings require both theory and practical portions to meet certification requirements. For these courses, we will teach the theory portion online via zoom, and the practical portion using our equipment on-site at the Training Academy, by appointment. These will be completed with a maximum of 2 students per group to allow for physical distancing and all equipment used will be cleaned between sessions.

Product Cleaning Guidance | Portable Gas Detection Instruments

For COVID-19, the United States Centers for Disease Control and Prevention (CDC) recommends the use of cleaning products with EPA-approved emerging viral pathogens claims. CDC refers to List N on the EPA website (https://www. epa.gov/pesticide-registration/list-n-disinfectants-use-against-sars-cov-2) for EPA-registered disinfectants that meet this guidance. Based on EPA guidance, the following cleaners may be used for all portable gas detectors that have the ALTAIR® branding, including the 4XR, 5X, 5X PID, 4X, 4XM, 2X, and single gas varieties:

Product Cleaning Guidance

  1. MSA Confidence Plus® 2, MSA part number 10009971. MSA’s Confidence Plus 2 cleaner is included on List N based on its EPA Reg. No., which begins with 47371-130.
  2. Disinfectants on EPA’s List N that meet the following criteria:
    1. Active ingredient of only Hydrogen Peroxide OR only Quaternary Ammonium and
    2. Do not contain alcohol or sodium hypochlorite (bleach) as an active ingredient
  3. If options 1 or 2 are not readily available in your market, you can use a 3% solution of Hydrogen Peroxide in water with 5 minutes of contact time on the surfaces.

Chemical Not Compatible with Products

Please note that for ALTAIR portable gas detectors, it is crucial that you do not use cleaners with chemicals that might damage the sensor performance or detector housings. This includes any disinfecting agents that contain alcohol or sodium hypochlorite (bleach).

Rigging in Space: SpaceX Uses Crosby 

Rocket science—Pushing the boundaries between new technology and old technology.

Whether you’re familiar with the ins and outs of rocket science or not, you probably have some idea of its complexity. Rocket science has even become slang for anything overly complex, detailed or confusing—With “It’s not rocket science” becoming a popular way of saying that something isn’t very complex.

But, did you know that even the rocket science industry depends on rigging hardware and technology in order to successfully see their work come to life?

SpaceX, Elon Musk’s Company, has gained worldwide attention for a series of historic milestones. It’s the only private company that has returned a spacecraft from low Earth orbit, which it first accomplished in 2010. The company made history again in 2012 when their Dragon spacecraft became the first commercial spacecraft to deliver cargo to the International Space Station.

Even a record-breaking company like SpaceX relies on the tried and true technology of rigging. Crosby wide body shackles were used on the recent SpaceX Starship lift near Brownsville, TX, to join the two halves of the spacecraft together.

SpaceX operates the world’s only operational reusable orbital-class rockets and spacecraft designed to carry both crew and cargo to Earth orbit, the Moon, Mars and beyond. Once complete, this star-ship will be the world’s most powerful launch vehicle ever developed. It will have the ability to carry in excess of 100 metric tonnes into Earth’s orbit.

Unfortunately that prototype, the Starship MK1, partially exploded during a pressurized test conducted by the SpaceX team late last month. It erupted at its tip, sending vapor and shrapnel flying into the air—You can check out the explosion here, captured on YouTube.

However, as stated by SpaceX a few hours following the test, “The purpose of today’s test was to pressurize systems to the max, so the outcome was not completely unexpected…There were no injuries, nor is this a serious setback.”

They’re wasting no time jumping into the next model, the Mk3, so there will surely be lots of record-breaking updates to come in this saga of futuristic space exploration!

For the past 17 years I’ve worked in managerial roles, with 14 years spent in the contact center industry.

Before I joined Hercules SLR, I worked as an Operations Manager for United Rentals, and before that I spent 10 years at VitalAire.

Why did you decide to work for Hercules SLR?

I decided to work for Hercules SLR because it was still a job that served the construction industry, which I find very interesting and challenging. And, the learning opportunities with Hercules SLR are endless!

I’ve been to Dartmouth a few times for training, but I’ve since taken on the new Territory Sales Manager role at Hercules SLR where I’ll see clients outside the branch—I look forward to hitting the road. meeting new clients and travelling to different cities in my territory.

What’s something you’re most proud to have accomplished in your career at Hercules SLR?

Within a few months, I was able to move into the Territory Sales position—I can’t wait to begin my journey, since sales have been a passion for me and I’ve been wanting to move into this type of role for awhile. I’m so thankful Hercules SLR could give me this opportunity!

What do you enjoy most about working in the rigging industry?

I enjoy the unlimited amount of product and knowledge there is to learn in the rigging industry, and the never-ending learning opportunities.

Why do you work safe?

I work safe because I have 2 children at home who require more than your average care, so it’s important for them to have the support & love they need and deserve. Coming home to them each day keeps me motivated.

I’m passionate about my health & always working to become a healthier, stronger & better me, so I can be there for my kids and have the energy to keep going, and feel GREAT while doing it!

I really admire my dad, because when he is faced with challenge or adversity, he faces it head on and works so hard to overcome them.

What’s your favourite family tradition?

My favourite family tradition is to have a “Secret Santa” gift exchange on Christmas eve, where we buy each other pajamas, make Christmas treats, and have hot chocolate. We watch Christmas movies together and it’s such a great time!

What is something you want to do next year?

Next year I want to have my wedding go off with no problems (and If you’ve ever planned one yourself, you know there’s always something that goes wrong!)

3M Fall Protection announces an immediate stop use and product recall of the 3M™ DBI-SALA® Nano-Lok™ edge and Wrap Back Twin-Leg Self Retracting Lifelines. The twin-leg Nano-Lok edge is part of a personal fall protection system and connects two self-retracting lifelines/devices (SRL’s/SRD’s) directly under the dorsal d-ring of a worker’s harness. It is intended to be anchored at foot-level, and is designed for sharp edge applications. The twin-leg Wrap Back Nano-Lok is intended for wrapping around an anchor and incorporates a similar energy absorber. 3M has determined that in the event of a fall and under certain conditions, the energy absorber may not properly deploy which could expose the worker to serious injury or death. Although there have been no reports of accidents or injuries associated with this issue, these products must be removed from service immediately.

To address this situation in the interests of worker safety, 3M is launching a global stop use of the Nano-Lok edge and Wrap Back Twin-Leg SRL units and is recalling all these units to be repaired or replaced as soon as a solution is identified, tested and certified. In the interim, and until a repair or replacement is available, end-users may elect to receive cash for their returned units as described below. This stop use and recall affects all versions of the Nano-Lok edge and Wrap Back Twin-Leg SRL’s since first introduced in 2013.

Users/Owners: Immediately stop using your Nano-Lok edge and Wrap Back Twin-Leg SRL and take these units out of service. Next, go to www. NanoLokEdgeRecall.com and follow the instructions on how to return your unit.

As soon as a product solution is available, 3M will either repair or replace your unit and return it to you at 3M’s expense. Alternatively, and until a product solution is available, you can elect to receive cash in the amount of $200 USD when you return your unit. As an interim solution for applications requiring 100% continuous connection in leading edge applications with sufficient fall clearance, 3M recommends the DBI-SALA® EZ-Stop Leading Edge 100% Tie-Off Cable Shock Absorbing Lanyard. You can also contact 3M Customer Service at 1-833-998-2243 or 3mcafpserviceaction@mmm.com for additional recall information.
